Updating of Particulars

FOR PRACTITIONERS WITH PROVISIONAL/ CONDITIONAL/ TEMPORARY REGISTRATION


1. Please note that your registration is granted in relation to your principal place of practice, therefore, you must inform the Board for any of the following changes: 

Scenarios

You are expected to do

Change of Practice Place including
change of outlet

1.Download and complete Form P2
2. Fill in your proposed practice place and appointed supervisor.
3. Even if the supervisor does not change, he/she must update their new practice place. Otherwise, your request will be rejected!
4. Email Form P2 two weeks in advance to OOB@spb.gov.sg  

Change of Supervisor

(Due to maternity leave/ long medical leave
- 1 month or longer)

1.Download and complete Form P3 
2. Fill in your proposed practice place and appointed supervisor.
3. Even if the supervisor does not change, he/she must update their new practice place. Otherwise, your request will be rejected!
4. Email Form P3 two weeks in advance to OOB@spb.gov.sg  

2. The Board will review your request upon receiving all the complete documents. 

3. While waiting for approval, you are to stop practising opticianry/ optometry at your new proposed practice place or under a new appointed supervisor until you have received the approval from the Board.

4. An email confirmation will be sending to you once approved.

5. Please be reminded that the Board has the power to review or cancel any registration if it is satisfied that the registered person has failed to comply with any condition to which his/her registration is subjected to.

FOR PRACTITIONERS WITH FULL REGISTRATION


Please note that under section 13(4) of the Optometrists and Opticians Act, all registered optometrists and opticians, whether practising full time or part time, must inform the Board in writing of any change in his name, residential address, practice address or other particulars as may be prescribed, within 28 days of the occurrence of the change.

Any registered person who have stopped practice, either temporary or for good, must also notify the Board in writing on the change of practice status within 28 days of occurrence of the change.

Any person who fails to comply may be compounded by the Board as an offence and shall be liable to a fine not exceeding $1,000.

Update Particulars Process  

1. Practitioners with full registration who wish to update their particulars on any changes to the following, please submit your update via Professional Registraton System (PRS).

2. With effect from 15 Jan 2017, all practitioners are required to log in PRS using Singpass & 2FA. Prior to proceed with your application, please refer to the process below.

For Non-Singpass User:
3. Practitioners who are holding a valid work permit but not eligible for Singpass, please contact the Board's secretariat if you are unable to log in PRS.

1. For Singaporean/ PR - log in PRS to update new address by attaching a copy of NRIC with new address
2. For work pass holder - email OOB@spb.gov.sg your new address

Work Permit/ S Pass/ E P (including re-issue with new pass)

1. Log in PRS and attach a copy of your new work pass
2. Log in PRS and attach a copy of blue NRIC for PR
3. Log in PRS and attach a copy of pink NRIC for new citizen

2. If it is a new optical shop, please arrange the employer to email a copy of business profile to open a new code
