Application for Restoration

Allied Health Professionals who were previously granted conditional registration and had your names removed from the Register pursuant to Section 26 of the AHP Act may apply for restoration of your names to the Register of Allied Health Professionals with Conditional Registration if you wish to return to practice in Singapore.

Please be informed that the prevailing supervision requirements will apply for your application for restoration.  This means that your prospective employer must be able to demonstrate its ability to provide the required supervision (i.e. completed the Readiness Review) under the Council’s supervisory framework and assigns you with a supervisor who meets the Council’s eligibility criteria for supervisors.

Submission of Application

The following documents must be submitted as part of your application for Restoration:

  • Statutory Declaration (to be duly signed and stamped by a Commissioner of Oaths or other officer empowered by law to administer Oaths) [PDF, 17KB];
  • Letter of offer of employment from the prospective employer in Singapore;
  • Letter stating grounds on which the application is made;
  • Details of employment history; and
  • Form SF2 - Undertaking by supervisor [PDF, 187KB].

Satisfaction of the criteria for restoration does not mean automatic approval. Each application for restoration will be considered on its own merits.

The application must be accompanied by a statutory declaration [PDF, 17KB] as set out in the Fourth Schedule of the Allied Health Professions (Registration and Practicing Certificates) Regulations 2013.  Please prepare the prescribed application fee of SGD200 payable by cashier's order or cheque only. From 1 October 2018, cheque should be made payable to "Ministry of Health (HQ)/AG".

The processing time for each application will take a minimum of 2 weeks, provided that the application and all required documents and information are in order as determined by the Allied Health Professions Council (AHPC).
