The Supervisor Nomination Form has been renamed to the Letter of Undertaking with effect from 7 October 2020.

The Letter of Undertaking for Conditionally-registered Dentists, Part II Oral Health Therapists and supervisors have been uploaded, and can be found at www.healthprofessionals.gov.sg/sdc/downloads.
