Singapore Nursing Board

1. Download and complete the Application for Transcript of Nursing Education (SON) Form.

2. The SNB Fee for Application for Transcript of Nursing Education (SON) is applicable.

3. You may make the Application either:

a) in person at SNB Operating Office.  Acceptable payment modes are:

• NETS, Cashcard;
• Cash;
• Cheque, Bank Draft or Cashier’s Order (Amount in Singapore Dollars, payable to “Singapore Nursing Board”)
  
  

b) by mail with payment and all required documents to SNB Operating Office.  Acceptable payment modes are:

• Cheque, Bank Draft or Cashier’s Order (Amount in Singapore Dollars, payable to “Singapore Nursing Board”)
  
  

Your application will only be processed after SNB has received the correct payment amount and the signed and completed Application Form. Incomplete Form will result in the delay of your application.
