Replacement of lost Certificates

If you have lost your Certificate of Registration/Enrolment or Practising Certificate (PC), you must apply for replacement IMMEDIATELY.

It is an offence to practise nursing/midwifery without a valid practising certificate.
[ Nurses and Midwives Act (Chapter 209) Section 26 (1) ]

To apply for a replacement, you must come personally to SNB Operating Office to make a report for the loss of certificate, providing information on when, where and how the certificate/s was/were lost and what efforts have been made to recover them (Attach a copy of the police report if it has been made).

The SNB Fee for Replacement of Certificate is applicable.
