1. If you are a locally trained nurse who have lapsed nursing practice for 5 years or more, you must attend and complete a Return-to-Nursing Programme.
2. If you are an internationally qualified nurse/midwife, you must have an offer of employment (on a case-by-case basis).
3. Certified true copy of the following documents will be required to support the application:
- NRIC/Passport
- SNB Certificate of Registration/Enrolment
- Current nursing licence (if applicable)
- Testimonials/References from previous employer(s)
- Certificate certifying the completion of a Return-to-Nursing Programme (if applicable)
Note:
i) Documents that are not in English mustl be accompanied by an accurate translation in English and certified by Justice of Peace, Notary Public, Commissioner of Oath from their country of origin.
ii) Documents certified as "true copies"
- Must be initialled on every page of the document by the authorized officer (Justice of Peace, Notary Public, Commissioner of Oath, your employer or your supervisor, unless otherwise stated.
- Must include the name of authorized officer (full name), signature, date of certification and healthcare institution stamp
iii) Photocopies of previously certified documents will not be accepted.
iv) Certified true copies will only be accepted in hard copy by mail or in person only (Fax or Email will not be accepted).
v) Copies which are not acceptable to SNB will result in the delay of your application.
4. If you have practiced nursing/midwifery in another country, you must provide evidence of registration in that country. You must also request for verification of your overseas registration to be sent directly to SNB.
5. You will need to submit a “Statutory Declaration for Re-registration, Re-enrolment or Re-certification Under Section 23” Form.
6. You will also need to submit two “Certificates of Identity and Good Character” each signed by a different person who must not be your family/relatives. These persons signing the Certificates should have known you since the time you were registered with SNB.
7. You must make an appointment with SNB to determine the requirements for your application and to obtain the relevant Forms. More information will be provided during your appointment with SNB.
8. The SNB Fee for Application of Re-registration/Re-enrolment is applicable.
Note:
SNB will not process your application until payment and all required supporting documents have been received. Failure to do so will delay your application.
