Criteria for Dental Specialist Accreditation

General Criteria

The criteria for dental specialist accreditation for the respective specialties under the non-grandfathering phase can be downloaded below:

• Endodontics
• Oral & Maxillofacial Surgery
• Orthodontics
• Paediatric Dentistry
• Periodontics
• Prosthodontics

Application for Dental Specialist Accreditation

Applicants should download the application form and send the completed hard copy with required hard copy submissions and the prescribed non-refundable application fee of $500 to the Dental Specialists Accreditation Board Secretariat.

Soft copy submission of the logbook should be in excel format and the rest of the documents in PDF format.

Information required as part of the application process:

• Name according to Identity Card or Passport
• DCR No.
• Contact details
  (both in Singapore and overseas, if abroad, at time of application. Please provide hand phone numbers and email addresses where available)
• Details of Basic and Higher Qualifications
• Details (date and specialty) of Exit Certification
• Name and contact details of Employers in Singapore
